Adults:  Financial information required for routine assessment appointment

1. Current driver's license or other state-issued identification
    
2. Social Security Card
    
3. Insurance Card (if applicable)
    
4. Proof of Residence (i.e. a utility bill in client’s name or a utility bill with letter from the person whose name is on the bill stating that the client lives in the home.)
    
5. Proof of Income (All Adult Residents). Non-working adults NEED a wage detail statement issued by THE TEXAS WORKSOURCE or bring proof of unemployment insurance income.
