- Supervised Living
- Foster/Companion Care
- Supported Home Living
- Personal assistance with daily living activities
- Assistance with medications
- Assistance with transportation to and from activities/appointments
- Habilitation, exclusive of day habilitation
- Facilitation of inclusion in community activities including social interactions
Eligibility Requirements
To meet the requirements, individual must be:
- Eligible for Medicaid or Supplemental Security Income
- Have a determination of mental retardation by a State of Texas licensed psyhologist
- Reside or be willing to reside within Montgomery, Walker or Liberty County
- Offered Home and Community Based Service (HCS) slot by DADS
